You randomly get them after dieing or after a server crash. Some of your items turn into question marks and most often remain unusable afterwards. The question marks usually don't sell to vendors or for 0-1c only. There's also a chance that instead of question marks your items just disappear.

Sometimes you still can equip the question marks, e.g. I was able to shoot mobs with them - these poor mobs must have died by confusion !

did u say keep the ?? I've had well over a hundred since the issue started. I have given up and started deleting everything, I have no room for the stuff that works tbh, I literally cannot carry more than 2 ?'s and my vaults are all full (cant ??'s be vaulted?). I am glad to hear there may be a fix, but I am 99% sure I did not see a dev post telling us to save our ??'s.
